1. Am I automatically enrolled in the pharmacy plan?

No.  Employees must fill out a health enrollment form to be enrolled in the pharmacy plan.  Also, enrollment in the health and pharmacy plans are not mandatory, however, health and pharmacy are a joint plan.  Employees cannot be enrolled in one without the other.

2. What qualifies me for the pharmacy plan?

Employees must be a regular full-time employee or full-time equivalent based upon Affordable Care Act (ACA) guidelines and be enrolled in the health plan. Contact Human Resources Benefits Office at 931-645-7421 for more information.

5. Can my dependents be covered on my plan?

Yes, eligible dependents include your spouse, unmarried, natural, legally adopted children, or stepchildren. The dependent child limiting age will be to age 26. Contact the Human Resources Benefits Office at 931-645-7421 for more information.

6. Can I enroll or make changes in coverage at any time?

No, changes are made during open enrollment or within 31 days of a qualifying event. Contact the Human Resources Benefits Office at 931-645-7421 for more information.

7. What should I do if my pharmacy deduction from my paycheck is incorrect or not being deducted at all?

Contact the City of Clarksville Payroll Office at 931-645-7442.  Gas & Water employees should contact the Gas & Water Payroll Office at 931-645-2565 x1107.  

8. When does my coverage end if I cancel?
